A quick look at a discussion on assessments that are commonly used within the ACT approach such as the AAQ2, the hexaflex assessment, GAD-7, PHQ-9, and in the progress based note the MMME and a functional assessment. In this discussion there is also some rationale for why you might include these assessments.
Regardless of what assessments you use, I would like to emphasize the importance of ongoing assessment in ACT, in that doing regular check-ins using help track progress and allow for adaptation of therapeutic strategies as needed. This is both useful for the client to help them notice any improvements as well as support the clinician in having a better sense that the work that they are doing is effective.
Not to mention, that having a good system in place has also been proven to be useful in having a case review with an insurance company, as I have found that once you can show that you are tracking this type of information, insurance companies tend to have case reviews a lot less often.
